I/we hereby certify that the information contained herein is complete and accurate. This information has been
furnished with the understanding that it is to be used to determine the amount and conditions of credit to be
extended, if any. Furthermore, I/we hereby authorize the financial institutions, trade references and all other
credit references in this credit application to release all necessary information to BLT Companies, LLLP in order to
verify the information contained herein and to establish credit with BLT Companies, LLLP. In the event a consumer
credit report is obtained on customer, the signature below authorizes BLT Companies LLLP to utilize the
information received to grant or deny credit to customer.
By signing and submitting this application, Applicant understands and agrees that all charges are due and payable in full
on the 10th day of the month following the date of purchase. Overdue accounts are charged a one and a half percent
(1½%) service charge per month, which equates to a rate of eighteen percent (18%) per annum. Applicant also
understands that in the event this account is not paid when due, Applicant agrees to pay all reasonable costs of collection,
including, but not limited to, attorneys’ fees and court costs.

Applicant agrees and understands BLT will not, nor is obligated, to sign any contracts or purchase orders generated by any general contractor, subcontractor or owner as BLT is only a material and/or equipment supplier. BLT’s credit application process is thorough and reviewed at the highest level which takes a minimum of 30 days to complete.

I/we, residing at (addresses) for and in consideration of BLT Companies, LLLP extending credit at my/our request to (company (hereinafter referred to as the "Company"), of which I/we am/are (titles), hereby personally guaranty payment on any credit
extended by BLT Companies, LLLP. Further, I/we hereby agree to bind myself/ourselves, individually and jointly and
severally, to pay BLT Companies LLLP on demand any sum which may become due and owing by the Company whenever
the Company shall fail to pay the same. It is understood that this guarantee shall be a continuing and irrevocable
guarantee, and indemnity for such indebtedness of the Company. I/we do hereby WAIVE NOTICE OF NON–PAYMENT AND
DEFAULT, AND CONSENT TO ANY MODIFICATION ON RENEWAL OF THE CREDIT AGREEMENT HEREBY GUARANTEED
